WEBSITE PRIVACY POLICY

CHA Hollywood Presbyterian Medical Center (“us,” “we,” or “HPMC”) is committed to respecting the privacy rights of its patients, customers, visitors, and other users of the HPMC Website (the “Site”). We created this Website Privacy Policy (this “Policy”) to give you confidence as you visit and use the Site, and to demonstrate our commitment to fair information practices and the protection of privacy. This Policy is only applicable to the Site, and not to any other websites that you may be able to access from the Site, each of which may have data collection and use practices and policies that differ materially from this Policy.

This Policy is incorporated into the Terms of Use posted on the Site. By using the Site, you signify your acceptance of this Policy and the Terms of Use. If you do not accept the terms of this Policy or the Terms of Use, do not access or use the Site.

Information Collection Practices

Personal Information We Collect

HPMC collects information about you, and sometimes about your devices, when you visit the Site. The information we collect about you through the Site generally is information that you provide or information that we automatically collect. For example, you may use the “Contact Us” feature on the Site to provide us your name and contact information. We also collect information about you, and sometimes about your computer or device, automatically through cookies and other technology. This is described in more detail below. We may collect information about your geographic location. We also may collect information regarding how you interact with the Site and on other websites, such as our social media platforms. For example, if you “like” a photo on one of our social media sites, we may collect information related to that interaction. In some cases, we may receive information about you from third-parties. Once we receive this information, we will use, disclose, and safeguard it as described in this Policy. We may combine information collected through different portions of the Site.

If we receive through the Site any of your personal health information, we will use and disclose such health information as described in our Notice of Privacy Policy, which relates to our collection, use, and disclosure of health information and which can be found here

Web Logs

We use web log information to make the Site more useful to visitors; to design a more positive user experience; to resolve user, hardware, and software problems; and for security purposes. All web logs are stored securely, and may only be accessed by HPMC employees or designees on a need-to-know basis for a specific purpose. We combine your web log information with the web log information of other Site visitors to create and then use the aggregated web log information to improve the Site.

Similar to most websites and best practices, we gather certain information automatically, and this information is stored for no longer than reasonably useful to carry out its legitimate business purpose, or as legally required. These logs may contain the internet domain from which you access the site (such as google.com, comcast.net, etc.), the IP address which is automatically assigned to your computer when you get on the internet, the type of browser and operating system you use, the date and time you visited, the pages or mobile screens you viewed, anonymous demographic data, and the address of the website you linked from, if any.

Internet Cookies

HPMC may place Internet “cookies” on the computer hard drives of visitors to the Site. Cookies help us obtain information about your use of the Site; they do not contain other information about you. HPMC uses two types of cookies: “session” cookies and “persistent” cookies.

A session cookie is temporary and expires after you end a session and close a Web browser. We may use session cookies to help customize visitors’ experiences on the Site, maintain a signed-on status while exploring the Site and track which Web pages visitors view on the Site. On the other hand, persistent cookies remain on your hard drive. Persistent cookies will not contain any personal information about you.

Use and Sharing of Information

We may use your information: to contact you (for example, sending you a newsletter or other informational materials, offers, announcements, or surveys); to communicate with you about our services and products, including to follow up on requests or questions that you may submit; to track and analyze use of the Site, including to prevent, detect, and investigate misuse, fraud, and illegal activities, and to track and evaluate activity on the Site; for purposes including enhancing and maintaining the Site, services, and products; to administer surveys and contests, as applicable; and to otherwise run our business.

We may share information that does not identify you with third-parties. For example, we use Google Analytics to process and analyze data we collect from your use of the Site. Additionally, we may share your information, including your information about you collected through the Site, under the following circumstances: with our third-party service providers who perform certain services or functions on our behalf (for example, we may share your information with a hosting service provider who hosts the Site); if a change in our ownership or control occurs, such as a sale or merger. If a sale or merger occurs, this Policy will remain in force; however, we may not have control over the new entity’s revision of the Policy after a sale); in accordance with your consent or direction, as permitted by law; as required to comply with applicable laws and legal process, including a law enforcement requests; to investigate and defend our and others’ rights and property (including intellectual property rights); and to protect the personal safety of people.

Data Integrity and Opt-out

If you receive a promotional email or newsletter from us, you can unsubscribe at any time by following the opt-out procedure within the footer of the email, as part of the CAN-SPAM Act. Please note, however, that you cannot opt out of certain messages, such as an email confirming your donation receipt.

Retention of Information

We may retain your information for as long as we deem reasonable or as long as required by law.

Security

The Site has security measures in place to prevent the loss, misuse, and alteration of the information that we obtain from you, but we make no assurances about our ability to prevent any such loss, misuse, to you or to any third-party arising out of any such loss, misuse, or alteration.

All of the information that you provide to us will be stored and processed in the United States. If you access our websites from outside of the United States, your use of the Site constitutes consent to the transfer of your information into the United States. The laws in the United States may not offer the same degree of privacy protection as the laws in your jurisdiction. We do not knowingly collect personal information about individuals residing in the European Union. Notice Concerning Children

We do not knowingly solicit or collect personal information from, nor does the Site provide content for, children under the age of 18.
