Chalet Phone: (323) 913-4830 Chalet Address: Chalet, 4636 Fountain Ave., Los Angeles, CA 90027 Hollywood Presbyterian Medical Center delivers comprehensive transitional and long term care at an off-site facility known as Chalet. Located adjacent to the hospital, Chalet offers 30 beds for patients needing transitional care services before returning home after a hospital stay. Wound care, rehabilitation and continuing IV therapy are common transitional care services. Wound care at Chalet includes use of a wound vacuum, a highly specialized machine that increases circulation and blood flow to the wound, thus allowing it to heal faster. Chalet also maintains 59 beds for patients needing long term care including those requiring ventilator support and other extensive respiratory and nursing services. Parenteral nutrition, a method of administering nutrition intravenously, is also provided for patients who are unable to eat on their own. For many long term care patients, Chalet becomes their home away from home. In addition to clinical care services, Chalet offers physical, occupational and speech therapies as well as such integrative therapies as massage therapy, music therapy and dog therapy. An on-staff activity coordinator uses games and other recreational activities to stimulate and enhance sensory awareness and coordination. Staff social workers help our patients cope with their conditions and bridge the gaps between patients and their families. From psychological therapy for those with long term illnesses to support for those trying to cope and/or prepare for death, social workers address and care for the emotional needs of our patients. To further ensure the delivery of quality care, Chalet hosts regular Family Counsels. These interdisciplinary monthly meetings with patients’ families serve to assess the patient’s status and identify ways to improve care. Patients are admitted to Chalet immediately following an inpatient stay at Hollywood Presbyterian Medical Center.
