Case managers use communication and available resources to promote quality cost effective outcome. Case management plays a key role in hospitals financial success. Hollywood Presbyterian Medical Center's case managers are all registered nurses who focus on planning and managing all aspects of patient services for their assigned physicians. Case managers works hand in hand with the interdisciplinary team members to ensure safe, appropriate and timely discharge, manage care to meet financial targets, and monitor patient transition and variation from plan of care across and within care settings. Case managers begin their discharge planning the moment a patient steps foot in the hospital. Case managers promote open communication amongst caregivers in order to achieve quality and cost effective outcomes. They make sure that case reviews are initiated within 24 hours of admission for medical necessity and InterQual criteria. Hollywood Presbyterian Medical Center's case managers possess a minimum of two years experience as a registered nurse in an acute care setting. Completion of case management certification course is required to be a case manager. Regular office hours are from 7:30 a.m. – 5:30 p.m. A case manager works on Saturday to deal with emergency discharge planning issues. On Sunday a case manager is on call from 8:30 a.m. – 5:00 p.m. for phone consultation on case management issues. The director and coordinator are available 24 hours a day 7 days a week for any case management concerns and /or questions.
