Documents to bring with you when applying for MEDI-CAL

If you don't have all of them we can help you!

1. Pregnancy Verification
2. California Identification Card or any type of identification card with photo
3. Alien Registration or Amnesty Card (both parents)
4. Social Security card from all family members
5. Birth Certificates from all family members
6. Most recent rent receipt, utility bills or a letter with your address
7. Verification of savings and checking (bring the most recent statements)
8. Most recent pay stubs (both parents)
9. If you are paid in cash, please provide a letter from your employer that states that amount paid and how often you are paid
10. Verification of employment or disability benefits
11. Verification of workmen compensation
12. Car registration
13. Vaccine cards and medical card (if you have received Medi-Cal before)
14. Marriage Certificate
